Hospice RN Case Manager – South Austin Area
$5,000 Hire Bonus ($1,500 at the end of 30-days, $1,500 at the end of 60-days, $2,000 at the end of 90-days).
Flexible Schedule: Work 4, 10-hour days or 5, 8-hour days.

What You Will Bring, Do, and Own
- The RN Case Manager will Complete assessments to determine Hospice needs.
- The RN Case Manager will Initiate and revise plans of care; regularly reevaluates patient, family and caregiver needs to incorporate palliative nursing interventions/actions.
- The RN Case Manager will Consult with and educate the patient/family regarding disease process, self-care techniques, end-of-life care, nutrition and dietary needs. Provide training to other staff as needed.
- The RN Case Manager Initiates communication with attending physicians, Hospice staff members and other facilities as needed to coordinate optimal care and use of resources for the patient and their family members.
- The RN Case Manager Monitors and maintains documentation of clinical records to ensure compliance with regulatory standards of timeliness, accuracy, and completeness via electronic record so threat problems, actions, and goals are accurately and clearly stated and changes are reflected as they occur, including new information obtained during Interdisciplinary Team meetings.
- The RN Case Manager Coordinates patient care for assigned caseload and provides direct care to patients maintaining the highest level of comfort and quality of life.
- The RN Case Manager Attends and provides support at the time of death.
- The RN Case Manager Participates in on-call duties as assigned.

What You Will Need To Have
- RN with unencumbered license in the state of Texas.
- Minimum 1 years’ experience as an RN in a medical/surgical acute care setting. Previous experience in Hospice or Home Health preferred.
- Comprehensive knowledge of pharmacology, palliative care, and end-of-life.
- Experience in analyzing processes, improvement planning, supporting change, and effectively managing priorities in a fast-paced environment.
Working Conditions and Physical Demands
You must be able to:
- Participate in physical activity.
- Work for extended period of time while standing.
- Lift up to 50 pounds and move patients.
- Ability to do extensive bending, lifting and standing on a regular basis.

Requirements
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements.
