Specialized Support Through Individualized Hospice Care Plans

The hospice philosophy is that the patient and their family are in the center of care. Creating an individualized hospice care plan involves understanding the unique needs of each patient and their family, and developing a care plan that is tailored to meet their specific needs and preferences. The goal is to provide comfort, support, and peace during this time, while respecting the patient’s wishes and maintaining their dignity.

At Blue Water Hospice, we also understand that medical conditions and related symptoms are very different. Our hospice team will conduct an initial assessment to evaluate a patient’s physical, emotional, spiritual, and social needs. This will help identify areas that need attention and guide the interdisciplinary hospice team in creating an individualized care plan.

Our Blue Water care team includes experts in various fields, including social workers, chaplains, physicians, and caregivers. We all work together to provide specialized support and care for the patient and their family, with the primary goal of ensuring that the patient’s final journey is as comfortable and dignified as possible.

As the patient’s condition evolves, the hospice care plan is dynamic and evolves, continuously changing to meet the patient’s most recent needs. The hospice care team will adjust the plan as needed to accommodate changes in the patient’s condition, so that the patient receives the optimal care and support at all times.
